What grinding does overnight
Grinding, clinically called bruxism, happens while you sleep, which is exactly why most people do not know they do it. It wears down the biting surfaces of teeth, and it can leave a jaw sore or a headache waiting in the morning with no obvious cause.
Often it is a partner who notices the sound first, or Dr. Saitta who spots the wear pattern on your teeth at a routine exam before you have connected it to anything yourself.

Custom fitted, not boil and bite
A drugstore boil and bite guard is shaped by hand and cools into whatever bite happened to be closed when you bit down. It is a rough approximation of your mouth, not a fit to it.
A custom nightguard is made from an impression of your own teeth, so it sits where it is supposed to and stays there through a full night, rather than working loose or feeling bulky enough that you stop wearing it.

Sports mouthguards, and why a custom one stays in
The same logic applies on the field. A stock or boil and bite sports mouthguard can shift or fall out during play, at exactly the moment it is needed.
